All referrals are accepted via fax. Please include a referral letter and any relevant test results.

What to Include in a Referral

• Patient demographics (name, date of birth, OHIP number, contact information)

• Reason for referral and clinical summary

• Relevant medical and surgical history

• Current medication list including any anticoagulants

• Recent lab work (especially creatinine, PSA if applicable, urinalysis, urine culture if applicable)

• Relevant imaging reports (CT, ultrasound, etc.)

• Indication of clinical urgency

• Bladder, kidney, ureteric, prostate, testicular cancers

• Kidney stones

• Benign prostatic hyperplasia (BPH) and lower urinary tract symptoms

• Urinary incontinence and overactive bladder

• Hematuria evaluation

• Vasectomy

• Adult circumcision

• Scrotal conditions

Wait Times

• Urgent referrals (suspected malignancy, severe symptoms): within 2 weeks (depending on urgency)

• Non-urgent / routine referrals: approximately 6-12 months

• All referrals are triaged based on the clinical information provided. If a patient’s condition changes,

please send an updated referral letter so we can re-triage
