Prior to your appointment, please complete the following:

Check your email for forms and requisitions: 

• You will receive your new patient questionnaire and lab requisitions via a secure Ocean email.

• If you cannot access email or print the requisitions, please contact our office to pick up paper copies in advance.

Complete requested lab work: 

• Please do this at least one week prior to your appointment.

Complete your voiding diary and new patient questionnaires (if applicable).

Prepare your medical history: 

• Bring a list of your medications.

• An itemized list of previous medical diagnoses and surgical procedures is also helpful.

Update us (for follow-ups): 

• Please inform us of any significant changes in your health since you were last seen by Dr. Dhanjani.

Cancellation and No-Show Policy

To ensure we can provide timely care to all patients, we require a minimum of two business days’ notice for all cancellations or rescheduling requests.

    •    Standard Appointments: Cancellations with less than two business days' notice or missed appointments (no-shows) will be subject to a $90 fee.

    •    Rebooking: Due to high demand, missed or late-canceled appointments are not guaranteed a new time slot. Rebooking is at the sole discretion of the clinic, based on clinical urgency and any documented extenuating circumstances. 

This policy is to ensure that we maximize our ability to accommodate all patients as no show appointments contribute to our wait lists. If  you have concerns about your ability to pay, please speak to our office about what options may be available - we do not intend to cause undue financial hardship, and you will not be denied care on the basis of your ability to pay.
